1. Event Disclaimer Form

Each student, Adult participant, advisor and volunteer must complete the event release and info sheet.  This form will provided emergency information for participants and fulfill our liability release requirements.

Please click on the link to the left to down load a printable PDF file.
2. Blue Merit Badge Form (you must provided)

This form is available from your scouting supply store or from your troop leader.
Please bring your form with you to the event, and fill it out prior to registration. 
The counselor will add his/her information and return form on Sunday.
3. EAA Young Eagles Form

At registration an original yellow form will be provided.  We would like very much to be able to scan and email this form to you. However, EAA has asked us to use all original forms.  Thus a Parent/Guardian must make arrangements to assist each youth at on-site registration or make other arrangements to pick up the form and return it to on-site registration.
THIS ORIGINAL YELLOW FORM MUST BE SIGNED BY PARENT/GARDIAN These forms will be available at drop of location or other arrangements can be made if needed to pick up form early and return day of event.  Contact Michael at michael@ae491.org or call 913-219-1370 to make other arrangements.
4. Youth Protections Credentials
(for Adults Only)

All Adults serving, participating or volunteering that will be interacting with youth participants must have completed youth protection training.  Please bring verification of this training.
